LOG IN PAGE

1. I forgot my password / I do not have a password

2. The Log In page is the page you should save in your Favorites, so you can find it again.

3. What should I expect when I log in?

You will see the Dacods Home page.
There will be some announcements and hints at the top.
This will be followed by a list of links to the Dacods forms.
There is a link to a page where you can search for Dacods records.
And there is a link to a page where you can see any of your Dacods that need some kind of correction.
There is a link at the very bottom to Colorado Health Partnerships.

PROBLEMS WITH ONE OF THE FORMS

4. When I click on the link to the form it does not open or the form does not seem to be working correctly.

You may not be “seeing” the latest version of the form or page. Use the F5 key to refresh you view.

To avoid having to use this key over and over:
On the very top of Microsoft's Internet Explorer click on Tools and then select Internet Options.
A small window will open.
Click on the "Settings" button (the first one, if there is more than one).
Close to the top, Find:
"Check for newer versions of stored pages"
and select:
"Every time I visit to the web page"
Click OK.
Click OK again.
On the very top of Mozilla Firefox click on Tools and then select Options.
A small window will open.
Click on the tab for Privacy.
Close to the middle, Find:
"Clear history when Firefox closes"
Check this if it is not checked
Click on "Settings" button just to the left
Check "Cache", "Form and Search History", "Download History", and "Browsing History" if they are not checked.
Uncheck "Saved Passwords" if it is checked.
Click OK.
Click OK again.
You now MUST close Firefox to delete your temporary internet files (cache).
And you should NOT leave Firefox open! Close Firefox EVERY day when you leave work!

5. I have tried using the F5 key, but this does not seem to work. Help!
You need to delete your temporary internet files (cache).

On the very top of Microsoft's Internet Explorer click on Tools and then select Internet Options.
In the middle of the window that opens find "Browsing history"
Make sure the the box "Delete browsing history on exit" is checked and click on the button "Delete".
In the window that opens find:
If you have Internet Explorer version 7.0
Find "Temporary Internet Files" and click on Delete.
Click on the button "OK".
If you have Internet Explorer version 8.0
Find and Uncheck "Preserve Favorites website data"
Find and check "Temporary Internet files", "Cookies", "History", and "Form data"
Uncheck the rest.
Click on the button "Delete".
Click "Close" at the bottom of window.
Click "OK" at the bottom of the window.
On the very top of Mozilla Firefox click on Tools and then select Clear Recent History ( Just above Options.)
In the window that opens, click on Details, so that they are visible.
Make SURE "Cache" is checked. Browsing and Download History and Cookies are OK to check too.
Just to the right find and click on the button "Clear Now".
Nothing will seem to happen, but the temporary internet files (cache) has been deleted.
Click on "OK".
Close and then reopen Firefox.


6. The Dacods form I sent in has information that is incorrect. What can I do?

If you sent in any form with the incorrect information, please complete and submit the correction form using the link below.

PROBLEMS WITH THE DISCHARGE FORM or the LIST OF OPEN ADMISSIONS

7. There is no Open Admission at all for my client or no Open Admissions that have the correct admission date on the List of Open Admissions. What should I do?

Be sure to check the OTHER list at the bottom of the page where Dacods waiting to be processed are shown.
If you cannot find the DACODS Open Admission on either list shown and you are sure it has not been already discharged:
Find your printed copy of the Admission. Make a copy of it and mail to the address listed above.
Please include a copy of the above Dacods Item Correction page with contact info, in case I need to reach you.

8. I cannot find the Open Admission I need because when I login, the Dacods Admission I am looking for has a different License Number and / or Program Number than I see just above the list of client names. What should I do?

You need a different login and password for every different License Number or Program Number you work with.
